Adult Camper/Youth Camper Health Form - TWO copies must be completed and brought to camp (for both Adult Campers and Youth Campers)
OMP Safe Sanctuary – Policy and instructions to complete training
Background Check Form – Submit and mail a $6 check to OMP. (Your form will not be processed until your payment is received.)
