OMP Participation and Release Form
Participant release forms are to be completed for all participants in an OMP mission week, both youth and adults. If under 18, please have youth fill out with a parent/guardian.

Forms for Group Leaders to Complete
Once you have registered, your group will need to complete these forms before April 1st. You can check the Group Leader Checklist below to make sure you stay on time for all required forms. Please submit forms using the corresponding links. All groups are required to submit the Background Verification Form and Group Roster by April 1st.
The Group Roster template will be emailed to youth workers once registered, or can be found below. If your group is attending more than one mission week, please submit a Group Roster for EACH mission week location separately.

VEHICLE TOOL LIST
Adult Community Builders serve as family group leaders. They will transport youth to and from job sites, participate in the projects and in camp activities, and will assist in maintaining the safety and positive atmosphere throughout the week.
If unable to provide all tools listed, OMP is able to supplement. Please reach out to janderson@ozarkmissionproject.org by April 1st if you need supplemental tools.
2 adults are required per vehicle. Vehicles can accommodate 3-10 youth depending on vehicle capacity. No more than 10 youth per vehicle.
